چکیده

ONE of the most difficult parts of orthotopic transplantation of the liver is construction of the upper vena caval anastomosis at the diaphragm. The problems are caused by the recipient disease. With chronic hepatic disorders, there often is foreshortening and distortion of the suprahepatic inferior vena cava as well as retraction of the main hepatic veins into the scarred liver (Fig. lA and B). Consequently, when orthotopic transplantation of the liver is performed, development of a long enough suprahepatic vena cava cuff for anastomosis to its homograft counterpart has ranged from trying to almost impossible. We report here in a technical solution which has been used in the last 12 instances. The host liver is devascularized and mobilized by techniques described before, including cross clamping of the infrahepatic inferior vena cava ( 1, 2). The suprahepatic vena cava is encircled and cross clamped, but no attempt is made initially to mobilize the desired length for cuff development (Fig. 1 A). Instead, an anterior vertical incision is made in the cirrhotic liver (Fig. IA), and with knife dissection, this is carried directly posterior to the vena cava which is scraped clean (Fig. 1 B). Vascular and Biliary Anastomoses in Deceased Donor Orthotopic Liver Transplantation

There are a total of five structures that have to be reconstructed in DDLT. These include the suprahepatic and infrahepatic vena cava, the portal vein, the hepatic artery and the bile duct. Prior to describing the various methods of implantation of the donor liver, we will briefly review the different options available for the recipient hepatectomy, as the surgeon’s choice of reconstruction of ...

Effect of Different Suprahepatic Vena Cava Reconstruction Methods on the Hemodynamics of Rats after Liver Transplantation

BACKGROUND There are few studies on the hemodynamic changes after orthotopic liver transplantation in rats. In this study, we aimed to evaluate the effect of different suprahepatic vena cava (SHVC) reconstruction methods on the hemodynamics of rats after liver transplantation. MATERIALS AND METHODS Three rat liver transplantation groups were created according to the SHVC reconstruction method...

Orthotopic Liver Transplantation in Rats

Clinical progress in the field of liver transplantation has been largely supported by animal models(1,2). Since the publication of the first orthotopic rat liver transplantation in 1979 by Kamada et al.(3), this model has remained the gold standard despite various proposed alternative techniques(4). Nevertheless, its broader use is limited by its steep learning curve(5). In this video paper,...

Surgical text for orthotopic liver transplantation model with small-for-size graft in the pig: key techniques and pitfalls

BACKGROUND Challenges with small-for-size grafts are a critical issue in the liver transplantation field, and a reliable and reproducible animal model is required. METHOD We performed 50 orthotopic liver transplantations in pigs with a 30% graft, and retrospectively investigated the learning curves. We modified our surgical procedures according to our experience. Here, we describe our current...
